Choosing a primary care provider is a personal decision...and we are grateful you're considering us. If you've decided Vitality is the right fit for your family, here's everything you need to make the switch official with your insurance.
How to Designate Us as Your PCP
Most insurance plans, including HMO, POS, and Medicaid managed care, ask you to formally name your primary care provider. There are two simple ways to do it:

Through your member portal. Log in to your insurance company's website, find the "Change PCP" or "Find a Doctor" section, and search by provider name or NPI.

By phone. Call the member services number on the back of your insurance card and ask to update your PCP.
